We assessed the changes in insulin secretory function and resistance in Korean from the 1990s and 2000s. Compared to subjects in the 1990s (n=578), those in the 2000s (n=504) exhibited increased insulin resistance and reduced insulin sensitivity, regardless of their glucose tolerance status. However, insulinogenic index was similar between two groups, and oral disposition index was lower in subjects with normal glucose tolerance in the 2000s than those in the 1990s. Experiencing high growth, changes in people's dietary patterns means increased hazards associated with increased insulin resistance regardless of the glucose tolerance status; however, the insulin secretory function remained unchanged, which resulted in increased susceptibility to the development of diabetes.

Among the 84 endogenous hyperinsulinemic hypoglycemia (EHH) patients who were diagnosed in a university hospital from 1998 to 2012, 74 patients were diagnosed with insulinoma, 5 with nesidioblastosis, and other 5 patients were diagnosed insulin autoantibody syndrome (IAS). Compared to insulinoma patients, who unequivocally showed fasting hypoglycemia, some cases of non-insulinoma did not show fasting hypoglycemia, but showed hypoglycemia during OGTT. All patients diagnosed with IAS showed severe hyperinsulinemia. Most patients with nesidioblastosis diagnosed before 2006, when we have started to measure insulin autoantibody, demonstrated severe hyperinsulinemia. Thus, prevalence of IAS may be higher in Korea compared to previous belief. Measurement of insulin autoantibody is warranted for EHH patients with very high plasma insulin levels.
 
Low economic status is identified as an emerging risk factor for diabetes mellitus in Korean men aged 30-59 years in Korean National Health and Nutrition Examination Survey 2008-2010. Diabetes Metab J. 2015;39:137-146.
 
We analyzed the association between economic status and the prevalence of diabetes mellitus in the Korean population between 30 and 59 years of age based on Korean National Health and Nutrition Examination Survey data from 2001 to 2010. In men, the prevalence of diabetes in the lowest income quartile was significantly higher than that in the other quartiles in 2008, 2009, and 2010 even after adjusting other metabolic risk factors, but not in 2001 or 2005. Although economic status was significantly associated with abdominal obesity, hypertriglyceridemia, and hypertension in women, there was no significant association between economic status and diabetes in women.
