Despite the demonstrated benefits of insulin therapy and existing recommendations in the guidelines for timely initiation of insulin therapy, evidence suggests that insulin utilization and glycemic control remain suboptimal. The MOdaliTy of Insulin treatment eValuation (MOTIV) study was conducted to assess the effect of insulin initiation in Korean patients who were inadequately controlled by oral antidiabetic drugs. In this real-world setting, the initiation of basal insulin provides clinically meaningful improvements in glycemic control and a well-tolerated treatment option in Korean patients with T2DM who are failing to meet targets with oral antidiabetic drugs.

Type 2 diabetes is a progressive disease. When the patient becomes hyperglycemic with submaximal sulfonylurea and metformin, dipeptidyl peptidase-4 (DPP-4) inhibitors could be added on. In this study consisting of 807 patients, we showed that triple combination therapy with DPP-4 inhibitors was effective. Rapid responders with ≥ 25% reduction in HbA1c within 12 weeks (from 10.1% to 6.9%, n = 66) were more likely to be men, had shorter duration of diabetes, and higher glycemic parameter at baseline. Women and the patients with lower body weight, triglyceride, and fasting glucose experienced more hypoglycemia. Proactive dose reduction of sulfonylurea could be considered when a DPP-4 inhibitor is added for rapid responders and hypoglycemia-prone patients.

Recent guidelines highlight that glycemic management should be individualized as multiple patients factors. Hypoglycemia is regarded as one of the major barriers to achieving glycemic control targets, and early detection of hypoglycemia is clinically important. In this study, we found that a history of cardiovascular disease was an independent risk factor for the development of severe hypoglycemia after adjusting for other related severe hypoglycemia factors (including cardiovascular autonomic dysfunction) in type 2 diabetes patients without renal dysfunction. Thus, for the patients with cardiovascular disease, modulation of glycemic target and intensive education is needed for the prevention of hypoglycemia.
