Sodium-Glucose Co-transporter 2 (SGLT2) inhibitors are promising anti-diabetic drugs with proven cardio- and reno-protective effects. We evaluated the predictive markers and the best combination therapy utilizing SGLT2 inhibitors in patients with type 2 diabetes. Our results showed that a better response is expected in patients with higher baseline HbA1c, high estimated glomerular filtration rate (eGFR) and a shorter diabetes duration. In addition, the add-on therapy of an SGLT2 inhibitor to baseline dipeptidyl peptidase 4 (DPP4) inhibitor yielded the greatest glycemic benefit.
 
Myricetin Protects Against High Glucose-Induced β-cell Apoptosis by Attenuating Endoplasmic Reticulum Stress via Inactivation of Cyclin-Dependent Kinase 5. Diabetes Metab J. 2019 Apr;43(2):192-205.
 
Myricetin is a natural flavonoid with therapeutic potential for diabetes. We investigated the effect of myricetin on high glucose (HG)-induced β-cell apoptosis and explored its underlying mechanism. We found that myricetin protects β-cells against HG-induced apoptosis by attenuating endoplasmic reticulum stress. This effect was attributed to inactivation of cyclin-dependent kinase 5 (CDK5) and consequent upregulation of pancreatic duodenal homeobox 1 and sarcoendoplasmic reticulum calcium ATPase 2b. The docking study predicted that myricetin inhibits CDK5 activation by direct binding in the ATP-binding pocket. These findings suggest that myricetin is a potential therapeutic agent for β-cell dysfunction.
 
The risk of myocardial infarction and ischemic stroke according to waist circumference in 21,749,261 Korean adults: A nationwide population-based study. Diabetes Metab J. 2019 Apr;43(2):206-221.
 
We studied the relationship between baseline waist circumference (WC) and development of incident myocardial infarction (MI) and ischemic stroke (IS) using a nationwide population-based cohort, and we evaluated if its predictability is better than body mass index (BMI) in the 21,749,261 Koreans who underwent the Korean National Health Screening. The incidence rate and hazard ratio of MI and IS increased linearly with WC, regardless of adjustment for BMI, and WC showed to be better in predicting cardiovascular disease (CVD) than BMI with smaller Akaike information criterion. The optimal WC cutoffs were 84/78 cm for men/women for predicting MI, and 85/78 cm for men/women for predicting IS. In conclusion, WC had a significant linear relationship with the risk of CVD. We found that the risk of cardiovascular disease began to increase even at a much thinner WC than we thought.
