Early Assessment of the Risk for Gestational Diabetes Mellitus: Can Fasting Parameters of Glucose Metabolism Contribute to Risk Prediction? Diabetes Metab J. 2019 Dec;43(6):785-793. |

Biochemical predictors of glucose metabolism and fasting indices of glucose homeostasis were assessed to predict gestational diabetes (GDM) and/or gestational diabetes with a need of pharmacotherapy (GDM-PT) later in pregnancy. Fasting plasma glucose (FPG) (ROC-AUC 68.1%), fasting insulin (FI) (ROC-AUC 67.2%), and fasting C-peptide (FCP) (ROC-AUC 70.6%) showed moderate to high accuracy in predicting GDM and GDM-PT. HbA1c was less accurate (ROC-AUC 65.3%), probably because HbA1c is a weak surrogate marker of insulin secretion and resistance. Additionally, quantitative insulin sensitivity check index, QUICKIc, was identified as the best predictor factor for GDM (ROC-AUC 83.7%) in the GDM-PT collective. Furthermore, FPG, QUICKIc at baseline and OGTT value at fasting were highly correlated with each other (r=0.48), but not with postprandial values (r=0.15). |

Short stature and leg length are associated with risk of diabetes and obesity. However, this association remains controversial and it is unclear whether this association is observed in Asians. Investigators studied new development of diabetes among 21,122,422 non-diabetic Koreans (mean age 43 years) enrolled in KNHS during median 5.6 year follow-up. The hazard ratio for diabetes development gradually increased from the 5th (reference) to 1st quintile group of baseline height after adjustment for confounding factors with statistical significance. In conclusion, height was inversely associated with diabetes risk in Korean adults. This association did not differ by sex, and was significant in lean individuals and those with normal FBG levels. |

The NOD-like receptor protein 3 (NLRP3) inflammasome is known to contribute to the progression of chronic kidney. The dipeptidyl peptidase-4 (DPP-4) inhibitors have shown protective effects on diabetic kidney disease, but the effect of gemigliptin on NLRP3 inflammasome activation in a model of renal fibrosis has not been fully investigated. We demonstrated that gemigliptin abrogated unilateral ureteral obstruction (UUO)-induced ECM accumulation. Furthermore, gemigliptin inhibited NLRP3 inflammasome activation and subsequent proinflammatory cytokine production in UUO kidney. The renoprotective effect of gemigliptin appears to be a consequence of down-regulated fibrotic gene expression via the suppression of TGF-β/NF-κB-induced NLRP3 inflammasome activation. |

Based on the previous findings that metformin plays a dual role in beta-cell dysfunction, we investigated the effect of low and high concentrations of metformin (0.05 and 0.5 mM) on beta-cell function under lipotoxic stress. Our findings reveal that metformin ameliorated palmitate-induced beta-cell dysfunction through a different mechanism depending on its concentration. High concentration of metformin activates AMPK, whereas low concentration of metformin, which is close to the concentration observed in the plasma of patients treated with usual clinical dose, suppresses ER stress and ROS levels elevated by lipotoxicity in an AMPK-independent manner. |

This study aimed to investigate the relationship between different obesity phenotypes and thyroid function in Korean euthyroid adults. A total of 13,873 adults from the sixth Korea National Health and Nutrition Examination survey (KNHANES) were included. Subjects were divided into four obesity phenotype groups based on body mass index and metabolic status. The present study demonstrates that serum free thyroxine (fT4) within the reference range was positively associated with metabolically healthy non-obese (MHNO) phenotype, although this association did not remain significant after adjustment for confounding factors. And thyrotropin (TSH) was negatively associated with MHNO phenotype. Higher normal serum fT4, but not TSH, was significantly associated with decreased prevalence of metabolic syndrome. |
